COMMUNITY CARE SERVICES PROGRAM (CCSP)

The Community Care Program assists older and/or functionally impaired consumers in remaining in their homes and communities. This Medicaid waiver program provides community-based social, health and support services to eligible consumers as an alternative to institutional placement in a nursing facility. Eligible individuals work with a care coordinator to develop a consumer-focused plan of care and arrange for delivery of services. Available services may include:

Home Health Services
Provision of nursing services, physical, speech, and occupational therapies; home health aides; and medical social services to individuals who meet the homebound criteria.

Alternative Living Services
State licensed personal care homes provide 24-hour supervision, medically-oriented personal care, and health related support services to those unable to remain in their homes.

Emergency Response Services
In-home electronic 2-way, 24-hour communication support systems call for help in an emergency.

Home Delivered Meals
Hot, nutritionally balanced meals are delivered to the home on weekdays. An individual must be eligible for additional CCSP services to participate in meal delivery.

Personal Support Services
Supportive services are provided in the home. These services include assistance and support with basic personal care needs, light housekeeping, and in-home respite services. Recipients of personal support services have the option to hire and supervise worker(s) of choice.

Adult Day Health
Provision of day-time care and supervision in an adult day center. Specific services include nursing and medical social services; planned therapeutic activities; physical, speech and occupational therapies; and meals, including prescribed diets.

Senior Citizen Centers
Local centers offer daily activities, meals and opportunities for social interaction.

Field Trips
Visit area landmarks, local restaurants, and other senior centers with your peers.

Home-Delivered Meals
For seniors who are unable to visit a center, our friendly drivers can deliver a healthy, nutritious meal to your doorstep 250 days a year.
